Description:  Percentage of patients aged 18 years and older with a BMI documented during the current encounter or during the previous twelve months AND with a BMI outside of normal parameters, a follow-up plan is documented during the encounter or during the previous twelve months of the current encounter. Normal Parameters: Age 18 years and older BMI => 18.5 and < 25 kg/m2

Initial Patient Population:  All patients 18 and older on the date of the encounter with at least one eligible encounter during the measurement period.

Denominator:  Equals Initial Population

Numerator: Patients with a documented BMI during the encounter or during the previous twelve months, AND when the BMI is outside of normal parameters, a follow-up plan is documented during the encounter or during the previous twelve months of the current encounter.

Inverse Measure:  No

Clinical Instructions

Track all 18+ year-old patients and ensure their BMI is greater than 18.5 and less than 25kg/m2. Any patient with a BMI outside of normal parameters requires a follow-up plan be documented during the current encounter or during the previous twelve months of the current encounter.

  •  While documenting the Visit encounter, record the BMI, as appropriate
  • Open the Vitals section and enter the Height and Weight (the BMI is auto-calculated)
  • If the BMI is out of range, the system will display a pop-up encouraging weight counseling education. Continue and precede to order the Dietary Consult, as needed
  • Open the Tests and Procedures section
  • Search for Dietary Consult in the autocomplete
  • Click Add to Exam
  • Open the Dietary Consult section
  • Complete and provide the follow-up plan, as appropriate
  • Close the section
  • Continue documenting the encounter, as needed
  • When completed, Close and Archive the encounter
